Request Form OL 392
Proof of Enrollment in Driver Training

If you are within Referral Driving School's service area (the Sacramento and surrounding cities area) you can use this online form to enroll in Referral Driving School and receive the DMV OL 392 form. As of January 1, 2004 the DMV OL 392 form is required in order for you to take your written test if you are under 18 years old.


Please fill out parts 1-4 completely and we will send you the OL 392 proof of enrollment form, a DMV application, and a list of everything that you will need to have with you when you go to the DMV to take the test. PLEASE NOTE: The OL 392 is a DMV form and it cannot be faxed or emailed, you must have the original. It is available through Referral only by completing this enrollment request and only by postal mail. It takes 3-5 days to receive the form after you submit. We are not able to provide it sooner.

Part 1: Student Information

Please make sure to use the full legal name of the student (as it appears on the birth certificate). The field "attending school" refers to the school the student is currently attending (this may be different than the school where the student received driver education).

Part 2: Driver Education Completion Certificate

Upon completing driver's education, the student should have received a certificate of completion (either form DL 387 or form OL 237). This section is referring to this certificate.

Note: The serial number is usually located on the top of the certificate, under the bar code.

Part 3: School of Driver Education

This section refers to the school where the student completed driver's education (and received the driver's education completion certificate). This may be different than the school that the student is currently attending.

This section refers to the address where we will mail the OL 392 form. This is most likely the home address of a parent or guardian.
